PARACardiac Arrest Best Practices
CPR Standards
- Rate: 100β110 BPM all ages; LUCAS device: 100 BPM
- Depth: 2 inches adult; compression fraction >80%
- Compressions FIRST β manage airway after 2 effective min
- All IV/IO drugs: follow with 10 cc NS bolus + elevate extremity
- Tidal volume β€500 cc in cardiac arrest; 1 breath q6 sec with advanced airway
- Nasal cannula Oβ 2β4 LPM during initial CPR
- After 2β3 min supine CPR: elevate head/shoulders ~30Β° over 4 min
- LUCAS placement must take β€20 seconds
Clinical Notes
Airway
- Continuous waveform capnography MANDATORY for all intubations and BIADs
- Use supraglottic airway device if intubation takes >30 seconds
- Oβ target: SpOβ >92%; use judiciously
- SpOβ monitoring: treat the patient, NOT the monitor
IV Guidelines
- Bolus = 20 cc/kg (neonates 10 mL/kg)
- TKO β 50 cc/hr (peds 5β10 mL/hr)
- AEMT: max 2 IV attempts one arm; no neck or leg IVs
- IO primary site cardiac arrest = Humeral Head (adults)
- IO primary site trauma = Tibial Plateau
- Lidocaine 2% 50 mg slowly before IO flush in conscious patients
Unstable (Symptomatic) β Any of the Following
- Chest pain
- Dyspnea
- Hypotension (SBP <90 mmHg in 70 kg patient)
- Signs/symptoms of CHF or pulmonary edema
- Signs/symptoms of MI
- Signs/symptoms of inadequate perfusion
- Altered level of consciousness
